Most of the following forms may be submitted online and a copy can be emailed to you for your records.  Some of the forms are also available in portable document format (PDF).  Adobe Acrobat Reader® software is required in order to view the PDF forms from your computer.  If you do not have Adobe Acrobat Reader®, click on the icon to the right to download a free version of the software.

Online submission or email between Sunday 1pm-Friday 1pm⇒response within 24 hours

Online submission or email between Friday 1pm-Sunday 1pm⇒response by Monday

For immediate correspondence, contact us between 9am-5pm, Monday-Friday

A doctor on call will return emergency messages left on the VNC recorder between 9am-5pm, Saturday-Sunday.

For emergencies between 5pm and 9am, Monday-Sunday, please contact your local Emergency Hospital.  If you do leave a message, we will contact you the following morning.
